Dear Parents/Carers
Attached is a letter from Oxleas NHS Foundation Trust. In preparation for the winter months and the holidays, Oxleas is offering an additional Covid-19 Vaccination Clinic at the academy on 23 November 2021.
Children will only be vaccinated with parental consent; Oxleas have provided the online eConsent form https://app.schoolscreener.com/Portal/#/Oxleas/imms/oxI I/141309 and have asked for this to be completed by Thursday 18 November 2021. A paper consent form is available from home school receptions if preferred.
The nurses will be working within the guidelines set out by Public Health England in relation to Covid-19 and the academy will continue to ensure your child’s safety at all times.
